SLEEP BRUXISM AND SLEEP APNEA: OVERLAP IN A SUB-GROUP OF PATIENTS

CATEGORY 2 - 3 CE POINTS
Biography:
GILLES LAVIGNE holds a DMD from the Université de Montréal and a PhD from the University of Toronto and is a Fellow of the Royal College of Dentists of Canada (FRCD in Oral Medicine; Georgetown University). He completed his postdoctoral training on the neurobiology of pain at NIH Bethesda. In 2009 he received an honorary doctorate from the Faculty of Medicine, the Centre for Dental and Oral Medicine and Maxillofacial Surgery, University of Zurich, Switzerland. He currently holds a Canada Research Chair in Pain, Sleep and Trauma. Dr. Lavigne served as Dean of the Faculty of Dental Medicine at the Université de Montréal (2008-2016) and President of the Canadian Sleep Society (2005-2006) and the Canadian Pain Society (2014-2016).
Course Outline:
Sleep bruxism (SB), or tooth grinding while sleeping, is a periodic rhythmic muscle activity. Its etiology may include regulation of anxiety, hypervigilance, and other emotional reactions and/or sleep arousal reactivity (cardio-respiratory), with some putative familial predisposition. SB patients do have more complaint of tooth damage, transient morning headache and orofacial pain.
Sleep apnea is characterized by repetitive cessation of breathing, reduction in oxygen (hypoxia), and higher morbidity risk (e.g. hypertension, diabetes, depression, vehicle accidents). Sleep apnea diagnosis is medical, but dentists recognize many risk factors in their practices: retrognathia, deep palate, narrow oropharyngeal space (Mallampati), overweight, sleepiness during their treatment, etc. The gold standard treatment is the use of CPAP, but dentists use mandibular advancement appliance, orthodontic or maxillofacial surgery to manage such condition.
The challenge for clinicians is to understand the interactions between these sleep disorders and to disentangle the causes in order to select the best management strategy. The differential of sleep disorders include isomnia, sleep-disordered breathing (SDB: snoring to sleep apnea) and/or movement neurological disorders (oromandibular myoclonus - epilepsy or REM behaviour disorder), and gastroesophageal reflux disease (GERD). Management strategies include sleep hygiene advice by a psychologist (cognitive behavioural therapies), medication prescribed by a physician (e.g. CPAP for apnea; muscle relaxant, analgesic, and antidepressives for pain or poor sleep complaints; proton pump inhibitor for GERD), sleep position trainer (to prevent stomach sleep) and oral appliances, e.g. occlusal splint for SB, manibular advancement appliance.

Your body is your home--the most important one you will ever live in. Feeding it well really matters, but, doing so is hard. Today's food landscape is littered with ultra-processed foods, stripped of their natural goodness, and often loaded with sugar and salt. We are bombarded with ever-changing, often conflicting, nutrition headlines and advice. Learning to make wise food choices; however, is worth it. Certain foods--berries, dark leafy greens, nuts, beans, turmeric, salmon, quinoa, and more--deserve superfood status. They significantly reduce the risk of heart disease, cancer, diabetes, and dementia. They also boost immunity, which includes a health microbiome. Most importantly, when you eat wisely and well, you greatly increase your ability to engage fully with each day. That is the power of good food.

WILLIAM H. HANG completing his orthodontic training at the University of Minnesota and then joined the faculty to teach orthodontics. After practicing traditional orthodontics for 7 years in rural Vermont, he saw serious limitations in esthetics and function with the traditional approach to orthodontics. He moved to California and developed an orthodontic practice with strong emphasis on facial esthetics achieved with innovative early treatment and adult treatment. He became aware of the positive effect some of his treatments were having on the airway. This opened a new aspect of orthodontics: maximizing the airway for all orthodontic patients and specifically treating patients with sleep disordered breathing (SDB) or obstructive sleep apnea (OSA). Dr. Hang's approach to orthodontics is a result of blending the best ideas from the best practitioners. He was the founding president of the North American Association of Facial Orthotropics, is a board member of the American Association of Physiological Medicine and Dentistry, and is an advisor to the Academy of Orofacial Myofunctional Therapy.
